The 18 categories of identifiers enumerated in HIPAA 45 CFR §164.514(b)(2) that must be removed for the Safe Harbor de-identification method to apply: names; geographic subdivisions smaller than a state; all elements of dates (except year) for dates directly related to an individual; telephone numbers; fax numbers; email addresses; SSNs; medical record numbers; health-plan beneficiary numbers; account numbers; certificate/license numbers; vehicle identifiers; device identifiers; URLs; IP addresses; biometric identifiers; full-face photographs and comparable images; and any other unique identifying number, characteristic, or code.
